This location provides the following comprehensive health care services:

- Emergency Dental Services
Most insurances are accepted, and no one will be denied care at Monmouth Family Health Center, because of an inability to pay. A Sliding Fee Discount Program (SFDP), that uses a sliding fee scale, is also available to all patients based on the individual’s ability to pay for the primary and preventative health care services rendered. This SFDP is available to all patients, and it is solely based on the individual patient’s income and family size. Please inquire about the program or make an appointment with the Financial Counselor on site.

Monday, Wednesday & Friday – 8:00 am to 5:30 pm
Tuesday & Thursday – 8:00 am to 6:30 pm
Every first and third Saturday of the month – 8:00 am to 4:00 pm

Directions to our 335 Broadway Location:
FROM NORTH NJ – Take NJ Turnpike to Exit 11 South to Garden State Parkway South. Head south and take exit 105. Stay on Route 36 Eastbound for approximately 3.4 miles and turn right onto Broadway. Travel 1.6 miles to 335 Broadway on your left.
FROM SOUTH NJ – Take Garden State Parkway North. Exit 105, then follow the directions above.
FROM WEST NJ – Take Route 195 East to the Garden State Parkway North. Exit 105, then follow the directions above.
From Route 18 and from the Garden State parkway southbound AND northbound.
