Most insurances are accepted, and no one will be denied care at Monmouth Family Health Center, because of an inability to pay. A Sliding Fee Discount Program (SFDP), that uses a sliding fee scale, is also available to all patients based on the individual’s ability to pay for the primary and preventative health care services rendered. This SFDP is available to all patients, and it is solely based on the individual patient’s income and family size.

FROM NORTH
Take NJ Turnpike to Exit 11 South to Garden State Parkway South. Travel 23 miles to Exit 105, stay on route 36 East for 3.4 miles then turn right onto Broadway. Travel 2.3 miles to make a right onto Ocean Ave. After 0.5 miles, make a right onto Pavilion Ave, we are located on the following cross street.

Take Garden State Parkway North. Exit 105, then follow the directions above.
FROM WEST
Take Route 195 East to the Garden State Parkway North. Exit 105, then follow the directions above.
