Nutritionist Services will no longer be offered directly by a Nutritionist at Monmouth Family Health Center (MFHC).  Your MFHC Primary Care Provider will provide you with nutrition wellness, education, and related services during your primary care visit, and may provide a referral for more advanced Nutritionist Services to a Nutritionist as needed.  For additional Nutrition Services contact

the Monmouth County Health Department at 732-431-7456

or the Visiting Nurse Association of Central Jersey at 855-619-4445.

NJ FamilyCare/Medicaid wants everyone to stay insured in New Jersey.

If you or your family get healthcare from NJ FamilyCare/Medicaid, we have important news about your benefits. NJ FamilyCare has returned to regular annual renewals.
From April 1, 2023 to March 31, 2024, NJ FamilyCare will mail the first renewal packets since 2020 to its members. Please make sure your mailing address is up-to-date with NJ FamilyCare by calling 1-800-701-0710 (TTY: 711). When you receive mail from NJ FamilyCare, make sure you follow the instructions and return your packet. If you have questions, visit nj.gov/StayCoveredNJ, or call 1-800-701-0710 (TTY: 711). Since it is part of our mission at Monmouth Family Health Center to make sure you are as happy and healthy as possible, our staff can also help you –
please contact any of our Financial Counselors at any of our three locations.
